Team Application Form

• This form must be received with the entry fee.
• The entry fee for all age groups is $650.00.
• The due date for applications is May 15, 2010.
• Postdated cheques will not be accepted.
• No refunds for teams that have been accepted into the showcase.
• There is a guarantee of 3 games to be played by each team.

Upon acceptance to the showcase, we will be requesting Player Profile forms to be filled out for each player. Please have your players fill these out and submit to Team Manager. Please send these profiles to us no later than June 6, 2010. Tournament Binders with this information will be made up and given out to all attending College Coaches

Coaches will be required to complete three copies of their team rosters, Click here to download the Team Roster Form. You will only need to complete the team rosters if your team is accepted into the showcase.

Travel Permit Information

Any Alberta teams attending the tournament will not require travel permits.  We would need a completed roster and the player cards would need to be available for every player on the team. 

All teams outside of Alberta will need to go through their Provincial/State organization body to obtain the necessary Travel Permits.
